Once your care is commissioned we can within 48 hrs provide an outline of care forming a bespoke reliable care team.
All our teams are led by a clinical lead nurse who are hands on and part of your care package. They will, uphold a quality of service and conduct timely, clinically assessments and create care plans addressing your complex of needs.
We provide a bespoke service to other providers, in bridging and managing the transition period. This is when clients are leaving acute hospital setting to a safe home environment.
